Social Worker – Māori Development Team & Liaison

About the role
Health and Connection supports the Mission by providing accessible, responsive, mobile and relationship-based health and social support to people experiencing significant barriers to accessing health care and other services.
The Social Worker – Māori Development Team & Liaison is responsible for providing professional social work assessment, intervention and advocacy alongside multidisciplinary coordination, liaison and referral oversight within the Health and Connection team.
The role supports equitable access to Health and Connection services by applying an equity lens to referral, triage and coordination processes, identifying barriers to engagement, and contributing to improved outcomes for Māori and other populations experiencing disadvantage.

What you will be doing
- Support multidisciplinary practice across Health and Connection, ensuring health, social, cultural, and whānau perspectives inform service delivery.
- Contribute to the multi disciplinary team hui, case discussions, and coordinated care planning to achieve positive outcomes for clients and whānau.
- Undertake social work assessments and develop strengths-based plans that reflect client and whānau aspirations, cultural values, and goals.
- Advocate alongside clients and whānau to address barriers to accessing health, social, and community support.
- Work collaboratively with internal and external partners to promote holistic wellbeing and service integration.
What we are looking for
- Qualified and registered Social Worker with experience supporting people and whānau in health, mental health, AOD, housing, homelessness, community, or other complex social service environments.
- Capacity to demonstrate through actions commitment to Te Tiriti o Waitangi and the Mission’s values of Manaakitanga, Atawhai, Rangapū and Mana Tika, Mana Ōrite.
- Demonstrated ability to work effectively across multidisciplinary teams and navigate multiple service systems.
- Strong assessment, care planning, advocacy, and relationship-building skills.
- Current full New Zealand driver's licence.
