Outreach Nurse – Nāhi Toro Kāinga Calder
About the role
At Calder Health Centre, Auckland City Mission Te Tāpui Atawhai, we believe everyone deserves compassionate, high quality healthcare, regardless of their circumstances.
We are looking for an experienced Registered Nurse to provide outreach based care to people experiencing homelessness, hardship and inequity across central Auckland. This is a meaningful opportunity to step beyond the clinic walls, build trusted relationships with whānau, and improve access to healthcare for people with complex health and social needs.
You will work both autonomously and as part of a multidisciplinary team alongside GPs, a Nurse Practitioner, Social Workers, a Health Coach and Housing teams. Together, you will deliver integrated, relationship-based care that supports people to live well, stay well and pursue their own hauora aspirations.

What you will be doing
- Deliver holistic nursing care in community settings using a trauma informed and culturally responsive approach.
- Build strong, trusted relationships with clients and whānau to support ongoing engagement with healthcare.
- Work closely with General Practice teams and other primary and community health providers to improve access and continuity of care.
- Coordinate referrals across health, social and community services, including specialist providers where needed.
- Support clients to build confidence, manage their own health where possible, and make informed decisions about their care.
- Use sound clinical judgement grounded in current nursing knowledge, evidence and reflective practice.
What we are looking for
- A current NZ Registered Nurse Annual Practising Certificate & at least 3-4 years’ recent nursing experience, ideally in primary care, community health or outreach settings.
- A demonstrated commitment to Te Tiriti o Waitangi and to the Mission’s values of Manaakitanga, Atawhai, Rangapū and Mana Tika, Mana Ōrite.
- Experience working with Māori and/or Pacific communities and with people facing complex health and social needs.
- Demonstrated ability to work with people experiencing mental health challenges, alcohol and other drug use, homelessness, hardship or high levels of social deprivation.
- A strong understanding of trauma-informed care and how to apply it in practice.
- A full, clean NZ driver licence.
